Ugh- wife and I paid off her car at the end of last year and got the title in the mail, with a note that said it needed to go into the DPS office to get the banks name off of it. My wife took it into the Williamson County office in Georgetown and they told her they'd need to take it, run it through some internal procedures, and they'd mail it back to us.

Well, we get to six weeks later and no title. We called them up and they said it was mailed over a month ago. The address they sent it to was correct and we've never seen it. I'm fairly sure its gone- either lost in their stupid office or lost in the mail. I'm not sure what issues this will cause (particularly regarding identity theft or if we ever decide to sell the car).

Anyone ever go through this or have any suggestions?

If the lienholder was taken off, all you have to do is apply for a lost title. It will be a certified copy and you will get it that day.
If the lienholder was not taken off, you will need a ROL(Release of Lien) letter from the lienholder, then you can apply for a lost title. Pretty easy and painless

So I don't have to worry about someone who may have plucked the original title out of the mail trying to do something with it (i.e. forge my signature and try to get the title in their name)?

Ha. It could happen, but you won't know until you get to the DPS or have someone that can run an MVR on your tags.

Yeah, just apply for another title, it will probably still have the lien on it, and you may have to get the lienholder to sign off on it again and repeat the process.
Anything someone does with your other title shouldn't be legal without your valid signature on it, but who knows-they potentially could use the title to get a loan at a bank or something.
It probably just fell through the cracks and won't cause any trouble for you.
